Volunteer Consent Policies and Procedures

As a volunteer for the Urban League I agree to abide by the policies and procedures. I understand that I will be volunteering at my own risk and that the organization, its employees and affiliates, cannot assume any responsibility for any liability for any accident, injury or health problem which may arise from any volunteer work I perform for the organization. I agree that all the work I do is on a volunteer basis and I am not eligible to receive any monetary payment or reward.

I understand that I am volunteering at the Urban League of Palm Beach County and that I will not be compensated for volunteering. I hereby release and hold harmless the Urban League of Palm Beach County, subsidiaries, officers, directors, employees, agents, successors and assigns (hereinafter “Releases”) from all claims associated with any injury sustained by me or to my property that may or later arise from my volunteering at the Urban League. I knowingly and freely assume all risks associated with my volunteering at the Urban League of Palm Beach County. I further understand and agree that the Release contained in this Agreement is intended to be as broad an inclusive as is permitted by the laws of the state in which the volunteering takes place and that if any portion of the Agreement is held invalid, it is agreed that the balance of it shall continue to be in full force and effect. By signing below, I understand, acknowledge, and agree that photographs, images or other likenesses of me and/or quotes attributed to me may be utilized in Urban League of Palm Beach County materials, regardless of their form, and I expressly consent to such uses (the “Uses”). I further understand, acknowledge, and agree that the Urban League’s right to make the Uses, either itself or at its direction, shall be royalty-free, perpetual, and worldwide, in addition, I waive any right to inspect or approve the Uses and release the Releases from any and all claims arising from the Uses. I further represent that I have the ability to make this agreement and that it does not conflict with any other agreement or requirement to which I am subject. I have read this Release and hereby agree that no oral representations, statements or inducement has been made and further acknowledge and agree that by signing my name herein I am volunteering at the Urban League of Palm Beach County voluntarily and of my own free will.
